Wegovy dosing schedule: 0.25 mg to 2.4 mg, and now 7.2 mg

Wegovy is semaglutide at weight-management doses. The current label (revised June 2026) covers an injection and a tablet, four indications, and three maintenance doses. This page walks the escalation table, then the choices at the top of it.

The escalation table

The label's Table 1 applies to every approved indication in adults and in adolescents aged 12 and older:

"If patients do not tolerate a dose during dosage escalation, consider delaying dosage escalation for 4 weeks." That is the only flexibility the table offers, and it runs in one direction. The titration planner dates each row from your first injection and lets you hold a step longer.

Maintenance by indication

Weight reduction (adults and adolescents 12 and older): 2.4 mg (recommended) or 1.7 mg once weekly, choosing on treatment response and tolerability. In adults, if 2.4 mg has been tolerated for at least 4 weeks and additional weight reduction is clinically indicated, the dose may be increased to 7.2 mg.

Cardiovascular risk reduction (adults with established cardiovascular disease and obesity or overweight): 2.4 mg (recommended) or 1.7 mg.

Noncirrhotic MASH with moderate to advanced fibrosis (adults): 2.4 mg. If not tolerated, decrease to 1.7 mg and consider re-escalation.

The 1.7 mg option is not a consolation prize. In STEP 6, run in Japan and South Korea, 1.7 mg produced 9.6% weight loss at 68 weeks against 13.2% for 2.4 mg and 2.1% for placebo (PubMed 35131037). The 1.7 vs 2.4 vs 7.2 mg comparison puts all three doses' trial data side by side.

Wegovy HD, 7.2 mg

Approved March 19, 2026 (Drugs@FDA supplement 29), the 7.2 mg dose comes in its own single-dose pen, 7.2 mg per 0.75 mL. In STEP UP, 72 weeks, mean weight change was 18.7% on 7.2 mg, 15.6% on 2.4 mg and 3.9% on placebo, a difference of 3.1 percentage points between the two active doses (95% CI 1.6 to 4.7), with gastrointestinal events in 70.8% versus 61.2% versus 42.8% and dysesthesia in 22.9% versus 6.0% versus 0.5% (PubMed 40961952). The label positions it as an option after 2.4 mg, not a destination.

The tablet

The Wegovy pill (oral semaglutide, approved December 22, 2025 per Novo Nordisk's announcement) has its own schedule in the same label: 1.5 mg daily for days 1 to 30, 4 mg for days 31 to 60, 9 mg for days 61 to 90, then 25 mg from day 91. Taken on an empty stomach in the morning with up to 4 ounces of plain water, then 30 minutes before food, drink or other oral medicines. The label gives switching rules: from 2.4 mg injection, start 25 mg tablets one week after the last injection; from 25 mg tablets, start 2.4 mg injection the day after the last tablet, or 1.7 mg injection if the 25 mg tablet was not tolerated. The pens

Five single-dose pens for the standard doses, each used once: 0.25 mg per 0.5 mL, 0.5 mg per 0.5 mL, 1 mg per 0.5 mL, 1.7 mg per 0.75 mL and 2.4 mg per 0.75 mL, four to a carton, plus the 7.2 mg per 0.75 mL Wegovy HD pen. Rules around the schedule

Same day each week, any time, with or without food; abdomen, thigh or upper arm. A missed single dose is taken if the next dose is more than 48 hours away and skipped otherwise; two or more consecutive misses raise the question of restarting escalation, which the missed-dose helper hands to your prescriber. The label says to stop Wegovy at least 2 months before a planned pregnancy because of the long washout.

Indications and the trials behind them

Weight reduction in adults (June 4, 2021) rests on STEP 1 to 4; STEP 1 alone showed 14.9% mean weight loss versus 2.4% at 68 weeks in 1,961 adults without diabetes (PubMed 33567185). Adolescents 12 and older were added December 23, 2022. Cardiovascular risk reduction (March 8, 2024) rests on SELECT. MASH (August 15, 2025) is an accelerated approval. Compounded semaglutide has none of this: no label, no escalation table, no indications. It is not FDA approved and not interchangeable with Wegovy.
