What the label says
Enter when the dose was due. The label rule for your product is applied to the time elapsed.
Semaglutide has a half-life of about a week, which is why a weekly injection works and also why a missed dose is rarely an emergency. The labels still give precise rules, and the two injection products give different ones. This page quotes them and the helper applies them.
Ozempic: the 5-day rule
Section 2.1 of the Ozempic label: "If a dose is missed, administer OZEMPIC as soon as possible within 5 days after the missed dose. If more than 5 days have passed, skip the missed dose and administer the next dose on the regularly scheduled day. In each case, patients can then resume their regular once weekly dosing schedule."
The clock starts at the missed dose. Day 5 is the last day to take it. Because the next dose is 7 days after the missed one, taking a late dose on day 5 still leaves 2 days before the next, which matches the label's separate rule that two doses must be at least 2 days (more than 48 hours) apart.
Wegovy injection: the 48-hour rule
Section 2.4 of the Wegovy label counts the other way, from the next dose:
- If one dose is missed and the next scheduled dose is more than 2 days (48 hours) away, take the missed dose as soon as possible.
- If the next scheduled dose is less than 2 days away, do not take the missed dose. Resume on the regularly scheduled day.
- If 2 or more consecutive doses are missed, resume dosing as scheduled or, if needed, reinitiate Wegovy and follow the dosage escalation schedule again, which may reduce gastrointestinal reactions on restart.
The first two rules are arithmetic and the helper applies them. The third is a prescriber decision, because whether to restart from 0.25 mg depends on how long you have been off and how you tolerated escalation the first time. The helper says so and stops.
Why the same gap gives different answers
Five days after a missed dose, the Ozempic rule still says take it (5 days is within 5). The Wegovy rule says the next dose is 48 hours away, which is not more than 48, so skip. The difference is where each label starts counting, not a difference in the drug. Follow the label for the product you were prescribed.
Tablets: skip
Every semaglutide tablet label has the same line: if a dose is missed, skip the missed dose and take the next dose the following day. That covers Wegovy tablets (1.5 to 25 mg), Rybelsus (3 to 14 mg) and Ozempic tablets (1.5 to 9 mg). Do not take two tablets in one day. The reason is the absorption window: the tablet has to be taken on an empty stomach with up to 4 ounces of water and 30 minutes before food, drink or other oral medicines, and a second tablet later in the day would not be absorbed the same way.
Compounded semaglutide
There is no label. A prescriber who writes a weekly compounded dose usually specifies which rule to follow; if yours did not, ask. The helper's Wegovy and Ozempic modes will apply either rule to your clock, but the choice between them is the prescriber's.
When not to dose at all
A missed dose sometimes happens because you were unwell. If you are vomiting, cannot keep fluids down, or have severe abdominal pain, the question is not whether the window is open. Both injection labels warn about dehydration leading to acute kidney injury and about pancreatitis, and both say to contact your prescriber. Questions people ask
I missed Wegovy on Monday and it is Friday morning. What does the label say?
Your next scheduled dose is Monday, about 72 hours away, which is more than 48 hours. The label says to take the missed dose as soon as possible, then resume on your regular day. By Saturday morning the gap falls under 48 hours and the label says to skip.
I missed Ozempic on Monday and it is the following Sunday. Take it?
No. Six days have passed, which is more than 5. The label says to skip the missed dose and take the next one on the regularly scheduled day, which is Monday.
Can I take two doses to catch up?
No label allows that. For tablets, the label says explicitly not to take more than one tablet a day.
