Label rule: Weeks 1 to 4 at 0.25 mg, 5 to 8 at 0.5 mg, 9 to 12 at 1 mg, 13 to 16 at 1.7 mg, then maintenance at 2.4 mg (recommended) or 1.7 mg. In adults, 7.2 mg is allowed after at least 4 weeks tolerating 2.4 mg. If a step is not tolerated, the label says to consider delaying escalation for 4 weeks.

Dates are computed on calendar days in UTC, so a clock change never moves a dose. Holds can only lengthen a step; the tool refuses anything shorter than the label minimum.

Assumptions, checked September 4, 2026

  • Wegovy injection: 0.25, 0.5, 1 and 1.7 mg for 4 weeks each, then 2.4 mg (recommended) or 1.7 mg; 7.2 mg in adults after at least 4 weeks tolerating 2.4 mg. Wegovy label revised 6/2026.
  • Ozempic injection: 0.25 mg for 4 weeks, then 0.5 mg; 1 mg and 2 mg each after at least 4 weeks at the previous dose and only if more glycemic control is needed. Ozempic label revised 1/2025.
  • Wegovy tablets: 1.5, 4 and 9 mg for 30 days each, then 25 mg. Rybelsus: 3 mg for 30 days, then 7 mg, then 14 mg if needed. Ozempic tablets: 1.5 mg for 30 days, then 4 mg, then 9 mg if needed.
  • Missed doses are not applied here; use the missed-dose helper. Compounded semaglutide is not FDA approved and has no label schedule; the planner only does calendar arithmetic for a schedule your prescriber wrote.
