What semaglutide costs in 2026: NovoCare self-pay prices, dated

Every price on this page has a date next to it because every price on this page will change. The numbers below are what Novo Nordisk published for self-pay patients buying through its NovoCare Pharmacy, read on September 4, 2026 from a price guide dated April 2026 and from the product sites. The tool above applies them.

Wegovy pens and Wegovy HD

Self-pay: $349 per month for the 0.25, 0.5, 1, 1.7 and 2.4 mg pens, where a month is one box of four single-dose pens. The 7.2 mg Wegovy HD pen is $399 per month.

New patients pay $199 for each of the first two months of the 0.25 mg and 0.5 mg pens, an offer Novo Nordisk states runs through December 31, 2026 and reserves the right to change or cancel. After that, $349.

Because the price is flat across doses, the cost per milligram falls as the dose rises: $349 buys 1 mg of semaglutide a month at 0.25 mg and 9.6 mg at 2.4 mg. That is a pricing decision, not a chemistry one, and it is why the cost view shows per-mg alongside per-month.

Wegovy tablets

$149 per month for 1.5 mg, $199 for 4 mg, $299 for 9 mg and $299 for 25 mg, a month being a bottle of 30. Wegovy.com listed 4 mg at $149 through August 31, 2026, then $199; the April price guide already shows $199. Since the maintenance dose is 25 mg, the steady-state tablet price is $299, which is $50 less than the 2.4 mg pen and $100 less than the 7.2 mg pen.

Ozempic pens and tablets

Pens: $349 per month for 0.25, 0.5 and 1 mg, $499 for 2 mg, with the same $199 two-month introduction on 0.25 and 0.5 mg for new patients through December 31, 2026. Tablets: 1.5 mg at $149 (listed for new patients), 4 mg at $199, 9 mg at $299.

The 2 mg Ozempic pen at $499 is the outlier. It holds 8 mg per pen (2.68 mg/mL, 3 mL) and delivers four 2 mg doses, so it costs $62 per mg against $36 per mg for the 2.4 mg Wegovy pen. Ozempic is labelled for type 2 diabetes, Wegovy for weight and cardiovascular risk; a prescriber choosing between them is choosing an indication, not a price.

Insurance and Medicare

For commercially insured patients Novo Nordisk's savings offer brings the copay to as little as $25 a month, subject to a maximum saving of $100 a month, for both pens and tablets; government beneficiaries are excluded. Wegovy.com describes a Medicare GLP-1 Bridge program at $50 a month for eligible Part D beneficiaries, effective July 1, 2026 through December 31, 2027. Terms are Novo Nordisk's and change; read the current page before relying on either.

Comparing per milligram

The custom mode in the tool takes any price, the total milligrams it buys and your weekly dose, and returns cost per mg, per dose, per month (52 weeks divided by 12) and per year. It is the only honest way to compare a monthly program fee, a box of pens and a compounded vial, and it is also incomplete: it prices the drug and nothing else. Compounded semaglutide is not FDA approved, is not interchangeable with Ozempic or Wegovy, and a low per-mg number tells you nothing about the pharmacy's licensure, testing or sterility. Price those second.
